In keeping with our core values, Central Montana Medical Center (CMMC) is committed to providing health care for people regardless of their ability to pay.

Assistance with medical bills - We recognize that medical bills may be difficult to pay.  If you are concerned that you may not be able to pay for all or part of your health care services, you may apply for financial assistance.  We can help you with the application and your personal information will be kept confidential.  If you have questions, please call the Patient Accounts Representative at (406) 535-6239, or toll free 1-800-732-5171, ext 6239.

CMMC’s financial assistance program has clear guidelines to determine who would qualify. The amount of financial assistance is different for each person or family, depending on their financial circumstances.

Return the completed form and necessary documents (see list below) to:

• Most recent Tax Return (Federal and State)
• Copies of earnings statements for each person in the household for the last three months (pay stubs, Social Security,
   unemployment, workman’s compensation, retirement, pensions, etc.)
• Copies of award letters for unemployment, food stamps, LIEAP, financial aid for college, or general assistance
• Copies of bank and investment account statements for the past three months
• Letter explaining your need for financial assistance
• Written verification from public assistance approving or denying assistance

Without the above listed items, CMMC may not be able to process your application.

Please return this application within 20 days from receipt of your bill for services at CMMC.  You will be notified in writing of any decisions made regarding your balance. You have the right to appeal any determination.
