CMMC’s Cardiovascular Disease & Diabetes Prevention Program is a group program supporting participants to incorporate healthy eating and physical activity into everyday life to help prevent Cardiovascular Disease and Diabetes.

The CMMC Cardiovascular Disease & Diabetes Prevention Program is an affordable, one-year program. For the first 16 weeks, group sessions will be held once a week. After the initial 16 sessions, classes will be held monthly. A daily log of exercise and food intake will be expected of all participants.
