The Home Health Program has been offering comprehensive, quality home care services since 1984. Those services available through CMMC Home Health are:

  • Registered Nurse: Visits to provide such services as Wound Care, Ostomy Care, Diabetes Education, medication education, assessment & education regarding the disease process and steps to recovery, patient & Family Education and Referrals to appropriate Therapies. Nursing services are available 7 days per week, from 8 am to 5 pm. 
  • Certified Nurse Aide: Assistance with bathing, medication reminders and other personal care needs.
  • Physical Therapy: Treatments and exercise programs to facilitate mobility, transfers and safety in the home.
  • Occupational Therapy: Provides retraining in vocational and leisure activities, activities of daily living, including toileting, dressing, self care and use of specialty equipment.
  • Speech/ Language Pathology: Provides therapy for swallowing, speech and cognitive difficulties.
  • Social Services: Evaluation of community resources and needs assessment. Counseling services for patients to help cope with emotional stress that frequently accompanies illness.
  • Registered Dietician: Offers dietary and nutritional counseling.

Medicare, Medicaid, Private Insurance, HMO & Workman's Compensation cover most Home Health services.

A Physician must order all services.
